To make the most of our day together, we ask that you provide key information about your church ahead of time. Completing this form will give us a clear picture of your mission, health, and opportunities so that our Strategic Momentum Process with your team is focused, contextual, and actionable.

Please complete this packet as fully as possible. If a question doesn't apply or you don't have the data available, simply note that.

Instructions

Assign one key leader to gather and submit the responses (though you may want to involve your team for accuracy).
